Enrollment Procedure

Freshmen and Transfer students are required to submit all needed documents(original & photocopy):

Freshmen Students:

  • Form 138
  • Good Moral Character Certificate
  • 2 pcs 2X2 ID picture
  • College Admission Test Result
  • NSO Birth Certificate
  • Police Clearance; and any of the following:
    • BIR Tax Exemption Certificate
    • Income Tax Return
    • Barangay Certificate of Indigence certified by the Municipal Social and Welfare Development(MSWD)
    • DSWD Listahanan certification
    • DSWD 4Ps certification

Transfer Students:

In addition to the above-mentioned documents except for Form 138, additional requirements are needed:

  • Honorable Dismissal
  • Transcript of Record/Copy of Grades
  • Subject Accreditation Form

Enrollment Flow

  • Step 1: STUDENT ENLISTMENT
    Students shall login to the student portal for the enlistment/registration and wait for the approval of their enlistment.

  • Step 2: STUDENT EVALUATION
    The Program Chairs/Department Chairs shall evaluate the credentials of new/transfer students or evaluate grades of returning/continuing students.

  • Step 3: SUBJECT ADVISING
    The Department Chairs/Program Chairs shall identify and approve the subjects to be enrolled by the students.

  • Step 4: ENROLLMENT
    The Registrar's Office shall do the confirmation of the subjects forwarded by the Department/College through the online portal.

  • Step 5: ASSESSMENT
    Immediately after the confirmation of the enrollment, assessment forms will be uploaded to the individual portal of the students.
